Landscape View Chart

I need to have a transcription of the attached chart done in Word as a table in landscape view, 8.5x11". Current sample is on one page -- but it can run to multiple pages, as long as you repeat the headers. Use similar coloring, but what I'm trying to do is to get legibility -- the current item is far too small. By using 8pt type but making each cell taller, we should be able to make it happen.
